
Affiliation Agreements
Often a student will want to work with an entity outside of UA who might request to establish an Affiliation Agreement with the University. This should only be done if you intend to work at that location while enrolled in the KIN 488 course. For more information, please email the company’s affiliation agreement to the Exercise Science internship Program Coordinator directly so
that it can be reviewed by the University’s legal team. See information regarding documents that may be required if working with
entities not affiliated with the UA campus. These documents may include:
- Official enrollment verification:
- The student can download this form located under “Official
Documents” on their student tab within My Bama. - Vaccination status, including COVID:
- The student health center or the student’s primary care physician
- Verification of liability coverage:
- The student must go the UA Risk Management link. Please note that this coverage will not extend to you
if you are working with another entity beyond the UA campus and are not registered for KIN 488. - An undergraduate student may request verification of liability coverage IF AND ONLY IF THEY ARE
WORKING AT THE OUTSIDE FACILITY AS PART OF THE KIN 488 CLASS. - Criminal background checks are not available unless paid for by the student; instructions are
posted in the button below for both in and out-of-state students. The University does NOT provide background checks or drug screening if the student is applying to work at a facility outside of the University. These services must be obtained by the student from an outside source.
